All paperwork must be received by 3:00 pm on the deadline date. Paperwork can be delivered to Shannah Wysocki in the Thunderbolt front office or sent by email (shannah.wysocki@lhusd.org).
Fill out the forms completed. If a question/item does not apply, mark it with "n/a".

- Medical Release form
- Concussion statement & acknowledgement form
- Physical (good for 1 year from the date seen by doctor)
