MANDATORY PAPERWORK is required to participate in school athletics.

All paperwork must be received by 3:00 pm on the deadline date. Paperwork can be delivered to the Thunderbolt front office or sent by email.

Fill out the forms completely. If a question/item does not apply, mark it with "n/a".

• Athletic Contract

• Athlete Emergency Card

• Proof of medical insurance

• Medical Release form

• Concussion statement & acknowledgement form

• Physical (good for 1 year from the date seen by doctor)
