Sports Registration Information
MANDATORY PAPERWORK is required to participate in school athletics.
All paperwork must be received by 3:00 pm on the deadline date. Paperwork can be delivered to the Thunderbolt front office or sent by email.

Athlete Emergency Card
Proof of medical insurance
Medical Release form
Concussion statement & acknowledgement form
Physical (good for 1 year from the date seen by doctor)
